Welcome to Active Rx Rehab & Wellness. These Terms of Service govern use of our website, online services, appointment-request services, SMS messaging program, and related communications.

Active Rx Rehab & Wellness provides physical therapy, rehabilitation, wellness, injury-prevention, performance, and related healthcare services.

Use of this website alone does not establish a provider-patient relationship.

3. Appointment Requests

Submitting an appointment request does not guarantee an appointment.

Appointments are confirmed only after communication with Active Rx Rehab & Wellness.

Appointments may be rescheduled or cancelled when reasonably necessary.

4. Patient Responsibilities

Patients are expected to provide accurate information, update relevant contact or insurance information, follow applicable care instructions, attend scheduled appointments or provide appropriate cancellation notice, and interact respectfully with staff and other patients.

5. Website Use

You agree to use this website only for lawful purposes.

You may not attempt to gain unauthorized access to systems, distribute malicious software, interfere with website operation, impersonate another person, or use website content unlawfully.

6. Intellectual Property

Website content, including text, graphics, branding, logos, photographs, videos, service descriptions, layouts, and other materials, is owned by or licensed to Active Rx Rehab & Wellness and is protected by applicable intellectual-property laws.

Consent to receive SMS messages is optional and is not a condition of purchasing goods or services, receiving treatment, or obtaining healthcare services from Active Rx Rehab & Wellness.

8. SMS Opt-Out

You may opt out of SMS communications at any time by replying STOP.

After sending STOP, you may receive one final message confirming your unsubscribe request.

Once your request has been processed, you will no longer receive messages from that SMS program unless you subsequently opt in again.

10. Message and Data Charges

Active Rx Rehab & Wellness does not determine or control charges imposed by your mobile carrier.

Wireless carriers are not liable for delayed or undelivered messages.

Delivery of SMS messages may be affected by mobile network availability, your wireless provider, your device, technical conditions, or other circumstances outside the control of Active Rx Rehab & Wellness.

Message delivery is not guaranteed.

12. SMS Privacy

No mobile information will be shared with third parties or affiliates for their own marketing or promotional purposes.

SMS opt-in information, mobile telephone numbers, and messaging consent will not be sold, rented, shared, or disclosed to third parties or affiliates for their own marketing or promotional purposes.

Information may be provided to service providers when reasonably necessary to operate our messaging, scheduling, communication, or healthcare-related systems on behalf of Active Rx Rehab & Wellness.

Text-messaging originator opt-in data and consent will not be shared with third parties for marketing or promotional purposes.

13. SMS Age Requirement

You must be 18 years of age or older to enroll in or use the Active Rx Rehab & Wellness SMS messaging program.

You may unsubscribe from promotional email messages using the unsubscribe mechanism contained in those communications.

I have seen Kyle to address my lower back and subsequent calf pain. He is very professional and knowledgeable and has various “tricks” to help alleviate the pain and address the root cause of my issues. It’s easy to see how educated and experienced he is, and he uses all of that within his treatment. I am getting better every day. Both Kyle and Lisa are also helping a family member with lower back pain along with plantar fasciitis. Along with their combined experiences, their kindness and understanding make a world of difference. I would definitely recommend them for anyone seeking help that hasn’t come yet. I was having knee pain from playing pickleball. Dr. Kyle performed dry needling on the knee and almost instantly the pain was reduced. In addition, he recommended several exercises and stretches I should use on a daily basis to increase strength and mobility!

First time having to do physical therapy for anything and it was for a frozen shoulder. Kyle and Lisa came up with an exercise program focused on the problem. After just a few visits my range of motion had already started to increase. They constantly added new exercises to continue to improve shoulder’s movement. I’m nearing the end of treatment and can honestly say my shoulder is near 100%. They were great to work with throughout this process, listened to concerns and would adjust exercises based off your current status. I know if I ever need anything like this in the future I’ll be in great hands with them.

I’ve been working with Dr. Kyle on mobility and strength training specifically for my golf game, and the results have been outstanding.
